Preparing For Your Appointment

Thank you for allowing us to take part in your health care. In order to best serve you, we ask that you please review the following information below prior to visiting our office.

Currently due to COVID-19, we have implemented the following screening and safety procedures in order to protect our patients and staff.

  • Facial coverings such as face masks are required to be worn in the building.

  • Temperature checks are performed and screening questions asked upon entry.

  • Additional cleaning with CDC recommended disinfectants is performed.

  • If you answer yes to any of the following, please call before coming to our office.

    • Have you tested positive for COVID-19?

    • Are you currently awaiting results from a COVID-19 test?

    • Have you had direct contact with someone quarantined or positive for COVID-19?

    • Are you experiencing any of the following symptoms, including fever of 100.4 or greater within the last 24 hours, new or worsening cough or shortness of breath, respiratory distress, nausea, diarrhea, vomiting, stomach pain, or loss of sense of taste or smell?

Please be aware that many of our patients experience allergic reactions to perfume and cologne; therefore we would appreciate your refraining from wearing any fragrances while visiting our office.

We ask that you please bring all of your medications to your appointment. The nurse and your doctor will review each medication to include the medication name, dosage, etc. at every visit as this is a very important part of managing your care.

Please remember to arrive 10 minutes before your appointment time if you are new patient in order to complete the nurse portion of your visit. Please let our staff know of any changes to your address, phone, and/or insurance at check-in.

If you need to cancel or reschedule your appointment, we require a 24-hour notice, or you will be charged a $50 fee.

Please bring the following with you to your appointment:

  • Insurance Card

  • Copayment, if required by your insurance

  • Picture ID

  • All Medications

  • Chest X-ray Films or CT Scan Disc if performed outside of a Novant facility; Please call our office to find out whether the location where your imaging was performed is one which are able to view at our office.
